honnou, supposedly, according to interviews with Ringo, was supposed to be about a women's right, separate and apart from man's, to a sexual fantasy. Ala the video was about her dressed up in fetish wear (nurse outfit) and having her own sexual fantasy (the scenes with her and the patient, this is also why the patient was a female and not a male, she didn't want to include men and water down the message) apart from men. Thus she was dressed as a nurse and running around not to please men but to please herself. People misunderstood the lyrics of the song (which are very sexual) coupled with the video and focused on the lesbian kiss, causing Ringo to be openly criticized and subjected to lots of negative media attention. This is why she felt so miserable during the SS era of her career, she felt most plagued by the "honnou" image of her and that is why she appears dead in the PV for Ichijuku no Hana.

For her to use the image of the nurse in the video would imply that the cosplay /nurse sex fetish is quite a common and large one in Japan. I don't think she could possibly hope that a video about her rubbing balloons on her body or being an amputee (both strange, but marketed, fetishes) could possibly carry that message to the public.

It's not like you know her enough to say that =P
But yeah, I suppose it would be incoherent with her career until Honnou.
I guess that media reaction actually traumatized her, that's why she now prefers to go the polite way. Any thought of going badass again makes her freak out.
